The development of oxytocic drugs in the management of postpartum haemorrhage
نویسنده
چکیده
Throughout history obstetric haemorrhage has been a major cause of maternal mortality in both developed and developing countries.1' 2The oldest work in obstetrics and gynaecology, published toward the end ofthe first century AD, is attributed to Soranus of Ephesus, who brought together the knowledge of obstetrics, gynaecology and paediatrics of that era.3 Soranus gives no account of oxytocic medications but does give some sensible advice on the general management of hyopvolaemic shock associated with postpartum haemorrhage, advising that the woman should 'lie down in a relatively small, dark and moderately cool room upon a hard bed ... raised a little at the foot'. He also described what could be regarded as the forerunner of the Medical Antishock Trousers (MAST): 'and the extremities should be gripped tightly and bandaged, for the compression resulting from this squeezing is transmitted all the way to the affected part'.'

The use of uterotonic drugs during caesarean section.
The administration of oxytocic drugs during caesarean section is an important intervention to prevent uterine atony or treat established postpartum haemorrhage. Considerable past and current research has shown that these agents have a narrow therapeutic range. A detailed knowledge by anaesthetists of optimal doses and side effects is therefore required. Oxytocin remains the first line agent. In...
متن کاملThe role of oral (methyl)ergometrine in the prevention of postpartum haemorrhage.
Postpartum haemorrhage (PPH) is one of the most important causes of maternal mortality in developing countries. A consensus was reached on active management of the third stage of labour for all parturients especially for those for whom the access to hospital services is difficult or time-consuming. Oral (methyl)ergometrine was considered to be a possible alternative prophylactic oxytocic, that ...
متن کاملPostpartum Haemorrhage: Still a Big Issue in Maternity Care - What is Going Wrong?
Background and aim: Postpartum haemorrhage remains one of the main leading causes of maternal mortality across the world. Reflecting on emerging evidence in this context, a discussion of associated factors which should be considered in interpretation of the evidence and its implications is presented ...

متن کاملStakeholder Analysis of Community Distribution of Misoprostol in Lao PDR: A Qualitative Study
BACKGROUND Globally, significant progress has been made in reducing maternal mortality, yet in many low-resource contexts it remains unacceptably high. Many of these deaths are due to postpartum haemorrhage and are preventable with access to essential obstetric care. Where there are barriers to access, maternal deaths could be prevented if community-level misoprostol was available. The purpose ...
